NHS/Social Care integration

Agincare's Live In Care service is purchased by the NHS (by Primary Care Trusts - PCTs) and by Councils as well as by private individuals. You might think that there is co-ordination between the NHS and Councils and, indeed, all do try to achieve this but amazingly people's care between hospital and returning home with on-going care needs is not really integrated there is an interesting article about this in the Community care magazine which might give you some interesting insights:
