FREE Live-in Care Needs Assessment

You are here

With Agincare, a care needs assessment is a free, no obligation process which is designed to assess the holistic needs of the service user. The assessment can be conducted within 24 hours of receiving the request and will be arranged at a time/place convenient to the client and/or service user.

A qualified assessor will arrive to conduct the assessment which will last approximately 1.5 hours. The assessment will then be used as the foundation to build the bespoke, person centred service you require. In order to progress any care enquiry, we suggest a care needs assessment is arranged as it allows us to provide all the facts and figures required for you to make an informed decision.

An assessment can be conducted at home, in hospital or in residential/nursing care setting. Please complete the form below to arrange a care needs assessment or to request further information.

This section will help us provide you with the best service if you could briefly tell us: Whether you are the person who wishes to receive the service or whether you are booking the assessment for someone else. It would also be helpful if you could summarise the reason you are considering live-in care, eg. is it because the care needs of the person wishing to receive the service have worsened, or is it because the person (or their family) wishes to have the peace of mind of having someone else in the house or just a wish for some companionship and minor support. You don't have to write a lot, in fact you don't have to write anything, but if you do, it will be useful.
