We are proud to announce that our Portsmouth D2A (Discharge to Assess) Team led by Sara Moncrieff are joint finalists alongside Portsmouth City Council in the 2017 Laing Buisson Awards. We have been jointly nominated in the Innovators and Leaders category for the Public/Private Partnership Award. The award recognises the achievements being realised in Portsmouth through integrated working and trusted partnership. This innovative pilot scheme is in the vanguard of services helping to reduce the growing pressures on the NHS and Social Care system.

We are jointly achieving outstanding outcomes for patients, helping to provide safe and timely discharge from hospital and preventing avoidable admission.

Before M died, he attended her in hospital, helping ensure the oxygen mask didn’t come off, including staying at her bedside overnight on one occasion. Last week, following M's passing , Ferenc was discretely around for my father - performing the ususal duties (meals, support getting in/dressed, medicines, dealing with visitors etc) but doing so in a way which didn't impinge on us as a family. Wanted to have conversations about M or just to have a quiet time with K. He judges very skilfully how much to be around and how much to leave us to our own devices.
