We are very proud of the achievements of our incredible Home First project teams who are finalists for the 2018 Laing Buisson Private/Public Partnership Award for the second year running.

Over the last 12 months, Agincare has been engaging nationally with Trusts, CCG’s and LA’s, forging proof of concept across a number of bridging and enhanced domiciliary schemes for a more radical approach to workforce challenges.

Key achievements

  • Reduced inequality of access
  • Increased admission avoidance
  • Improved patient flow
  • Reduced stranded patients & DToC
  • Intelligence-led discharge planning

In the winter of 2017/18, we mobilised a number of urgent homecare capacity surges, these have all converted into long-term sustainable models in preparation for this coming winter.  Each has positively impacted on acute and community systems, providing essential breathing space to normalise pressures.

This year’s award nomination features our partnership with the Royal Bournemouth and Christchurch Hospital (RBCH) Trust team as an exemplar of this high impact model.

The Home First vision of Cherry McCubbin (RBCH Associate Director) and Val Horn (RBCH Discharge Manager) was planned alongside the three involved local authorities, the CCG and Agincare. Over the course of 2018, the scheme has evolved, scaling capacity up and down to suit demand, shifting from a classic bridging service to an enhanced model.  Throughout, our fantastic team led by Dawn Robinson, have been incredibly responsive and flexible, role modelling a more robust approach to getting people home from the hospital.

Over the last 12 months, it has been our privilege to step up and stand alongside our public sector partners. This award recognises that innovation and a ‘can do’ attitude can deliver the real transformation of outcomes in systems under pressure.
